Please Note: The surgery will close at 1pm on Wednesday 27th September 2017 for essential staff training. The Surgery will re-open at 8.00am on Thursday 28th September 2017.


There are some changes to our telephone system.


As of 31st July 2017 an additional telephone number will be available for patients with prescription queries.  To book an appointment please call 0151 495 5110 from 8am onwards.  No prescription queries will be dealt with using this appointment line.

 

You may print off a registration form, fill it out and bring it in with photo ID and proof of address on your first visit to the practice.  We can then get you registered on to our clinical system.  We are also able to sign you up for online services at the same time.  Ask our receptionist for further information with regards to online services.  Alternatively you can use the link below to complete your registration online
